What counts as a complex orthodontic case?
Significant skeletal discrepancies — cases where the upper and lower jaws do not relate correctly in size or position — may require orthodontic camouflage using TAD mechanics, or combined orthodontic and surgical treatment. The decision between these two pathways depends on the severity of the discrepancy, the patient's facial aesthetics, and their willingness to consider surgery. Impacted teeth — teeth that have failed to erupt and remain embedded in the bone or gum — require careful biomechanical planning to expose and guide into position in the arch. This often involves coordination with an oral surgeon for surgical exposure, followed by orthodontic traction using TADs or fixed appliances over an extended period.
Severe crowding requiring extractions demands careful treatment planning to ensure spaces are closed in the right direction, with the right teeth moving in the right sequence.
